Dental membership plans: steady income from patients without insurance
Half the patients in most dental clinics in Egypt and Saudi Arabia pay cash. They have no insurance, or their insurance does not cover dental, or the coverage is so limited they stop using it. These patients come when something hurts, pay for that visit, and disappear until the next problem.
A membership plan changes that pattern. The patient pays a fixed amount — monthly or annually — and gets preventive care included plus a discount on everything else. The clinic gets predictable revenue and a patient who visits twice a year instead of once every three years.
This is not a loyalty programme. Loyalty programmes reward visits that already happened. A membership plan is paid in advance and creates the visits.
How it works
The patient pays an annual fee (or twelve monthly payments). In return:
- Two checkups and two cleanings per year, included
- One set of bitewing X-rays per year, included
- A fixed discount — typically 15–25% — on all other treatment
- Priority booking or emergency access, if the clinic offers it
The clinic sets the price so that the included services roughly break even at the clinic's cost (not its list price), and the discount on other treatment is the incentive to come back for the crown or the filling rather than going elsewhere.
Why it works for the clinic
Predictable revenue. 200 members at 1,200 EGP per year is 240,000 EGP the clinic can count on, paid in advance or in instalments. It arrives whether or not those patients have a toothache.
Higher visit frequency. A member who has already paid for two cleanings comes for both. A cash patient skips the second one. Each visit is a chance to find and treat something.
Higher treatment acceptance. A patient who has a 20% discount on a crown is more likely to say yes than one paying full price. The clinic earns less per crown but does more crowns.
Lower marketing cost. Members do not need to be re-acquired. The clinic spends marketing money on new patients, not on reminding old ones to come back.
Why it works for the patient
The patient gets a lower total cost of dental care, a reason to come for preventive visits (they already paid), and no surprise bills for routine care. For families, a family plan at a lower per-person rate is the easiest sell in the clinic.
Pricing for Egypt and KSA
Start from the clinic's actual cost of the included services, not the list price.
| Component | Egypt (EGP) | KSA (SAR) |
|---|---|---|
| 2 checkups + 2 cleanings (clinic cost) | 600–900 | 300–450 |
| 1 set bitewings (clinic cost) | 100–150 | 50–80 |
| Margin + admin | 300–450 | 150–220 |
| Annual membership price | 1,000–1,500 | 500–750 |
| Monthly equivalent | 85–125 | 42–63 |
These are illustrative ranges. The right number depends on the clinic's costs, its location, and what competitors charge for a single cleaning. The test: the annual fee should be less than what the patient would pay for the two cleanings alone at list price, so the maths is obviously in their favour.
Family plans: 20% off per additional member. Children under 12: 50% of the adult rate.
What to include and what to leave out
Include: Checkups, cleanings, routine X-rays, fluoride for children, a discount on all other treatment, emergency visit at no charge.
Leave out: Anything with a lab cost (crowns, dentures, aligners), surgery, cosmetic work, and anything the clinic refers out. These get the member discount but are not included.
Decide case by case: Simple fillings. Some clinics include one or two per year; most apply the discount instead. Including them raises the price and makes the plan harder to explain.
The three mistakes that kill membership plans
1. Pricing it as a discount card.
If the plan is cheap and includes nothing, patients buy it and do not change their behaviour. The included cleanings are what make them come back. Price the plan high enough to include real care.
2. Not tracking who used what.
If the clinic cannot see that a member has used one of their two cleanings, the member either gets a third for free or gets charged for the second. Both break trust. The clinic system has to track plan benefits per member per year.
3. Not reminding members to use it.
A member who paid for two cleanings and used none feels cheated at renewal time, even though it was their choice. The clinic has to send the recall. "You have one cleaning left this year — book it before December." Renewal rates double when members used what they paid for.
Launching the plan
Week 1: Set the price, write the one-page plan description, build the benefit-tracking in the clinic system.
Week 2: Train the front desk on the pitch. The pitch is one sentence at checkout: "Did you know that for [price] a year, your two cleanings are included and everything else is 20% off?"
Week 3: Offer it to every cash patient at checkout. Target: 10% take-up in the first month.
Month 3: Review. How many members, how many used a benefit, what is the average spend per member versus non-member cash patient.
Month 11: Send renewal reminders. Members who used both cleanings renew at 80%+. Members who used none renew at under 40%.
What Dentolize does
Dentolize tracks membership benefits per patient: what is included, what has been used, what remains. When a member books a cleaning, the system shows the receptionist "Membership: 1 of 2 cleanings used" and applies the right price automatically. The member discount applies to the invoice without manual adjustment. Renewal reminders go out by WhatsApp sixty and thirty days before the plan expires, listing the unused benefits.
The dashboard shows membership revenue separately from fee-for-service, so the clinic can see the predictable portion of its income.
Start with one number
Count your cash patients from the last twelve months. If it is more than 100, a membership plan is worth building. If 15% of them join at 1,200 EGP, that is 18,000 EGP of predictable annual revenue from one month of pitching at checkout — before counting the extra cleanings and the discounted crowns.
Dentolize includes membership plan tracking with automatic benefit application and renewal reminders. Request a demo.
