Consent forms

Digital dental consent forms, signed and stored

A consent form is worth exactly as much as your ability to produce it, in the version the patient signed, on the day they signed it. Paper in a drawer fails that test in the only situation where it matters.

Signed where the patient already is

Consent taken on a tablet at the chair, or sent ahead as a link so the medical history is filled in before the appointment starts, is both faster and more complete than a clipboard.

  • Signed on a tablet at the chair or on the patient’s own phone
  • Sent ahead of the appointment, so the chair is not used for admin
  • Arabic or English, following the patient record
  • Required fields enforced, so a form cannot be half-completed
  • Signature captured and stored with the document

Versioned, because forms change

The consent a clinic uses today is not the one it used two years ago. What matters at a dispute is producing the version in force at the time, with what the patient actually answered.

  • Every form versioned, with the version recorded on each signature
  • Timestamp, patient, doctor and treatment stored with it
  • Answers kept as data, not as an image of a page
  • Nothing edited after signing

Forms beyond consent

The same mechanism covers everything a clinic needs signed or answered: medical history, post-operative instructions, treatment acceptance, photography permission.

  • Custom forms built per clinic without waiting for a release
  • Medical history answers flowing into alerts on the chart
  • Post-operative instructions sent over WhatsApp after the visit
  • Anything signed retrievable from the patient record in seconds

Who relies on consent

Consent protects the clinician and informs the patient, and it is administered by whoever is at the desk.

Dentists

Treats knowing the medical history was answered and the consent is on file, in the right version.

Front desk

Sends forms ahead instead of handing over a clipboard.

Owners

Can produce any signed document, with its version, without searching a cabinet.

By hand, and with Dentolize

Paper consent is fine until somebody asks for a specific form from a specific date.

Doing it by hand

  • Paper in a filing cabinet
  • Whichever version was printed
  • Answers on a page nobody reads
  • Filled in at the chair
  • Retrieval measured in minutes

With Dentolize

  • Stored against the patient record
  • The version in force, recorded on the signature
  • Answers as data, feeding chart alerts
  • Sent ahead and completed before arrival
  • Retrieval measured in seconds

Questions

Consent forms — frequently asked

Are digital consent forms legally valid?

Electronic signatures are widely accepted, and a digital record is generally easier to defend than paper because it carries the version, timestamp and answers. Confirm the requirements that apply in your country.

Can patients complete forms before the appointment?

Yes, sent as a link over WhatsApp, which also means the medical history is available before the patient arrives.

What happens when we change a form?

The form is versioned. Existing signatures keep the version they were signed against, so you can always produce what the patient actually agreed to.

Can we build our own forms?

Yes, per clinic, including medical history and questionnaires, without waiting for a software release.

Do medical history answers reach the chart?

Yes. Allergies and conditions raise alerts that follow the patient into every screen.
