Free documents

Free dental forms and records

The clinical records a dental clinic keeps on paper, laid out around the tooth chart. Word files to put on your letterhead and edit, and a PDF of each to print as it is.

How the documents are made

Word to edit, PDF to print

Each document comes as a Word file with a letterhead block for your logo and details, and as a ready PDF.

Arabic and English

A separate file for each language, laid out right to left or left to right, not one page with both squeezed in.

FDI numbering

Charts use the FDI two-digit system, quadrant first, as most clinics in the region and in Europe do.

Questions

Are the documents really free?

Yes. Download directly: no form, no account and no watermark.

Can I add my logo?

Yes. Open the Word file and replace the logo box at the top with your own; the layout keeps its size.

Are these a substitute for clinical judgement?

No. They are a structure for recording what you find and decide. What to record, and how, remains the dentist’s call.

Is there a way to fill them on screen?

Several have a free tool that fills and prints them in the browser on your letterhead. Each document page links to its tool where there is one.

Want these on the patient’s chart?

Findings, perio charts and treatment notes that live in the patient file, on the tooth they belong to. Thirty minutes on your own clinic.
