Dental consent forms: what the patient should sign before an extraction, an implant or braces
A patient complains a year later of numbness after a wisdom tooth extraction. Was a consent form signed that mentioned this possibility? And where is it? In most clinics the answer is a general form signed at first registration that does not mention extraction, or a sheet in a file nobody can find. A consent form is not paperwork for the inspector; it is what protects the clinic and the patient together when what can happen, happens.
A form per procedure, not one form for everyone
On the general registration form the patient consents to "treatment". That covers nothing. Informed consent means the patient understood the specific procedure, its risks, its alternatives and its cost, and signed for exactly that.
In a dental clinic that means separate forms at least for surgical extraction, root canal treatment, implants, orthodontics, whitening, anaesthesia, x-rays for pregnant patients and paediatric procedures (signed by the guardian). Each has its own content: numbness for extractions, the possibility of implant failure, the length of orthodontic treatment and what happens on drop-out.
What the form must contain
A description of the procedure in the patient's language, not the dentist's; the common risks and the important rare ones; the alternatives, including no treatment; the cost and what it excludes; after-care instructions; what the patient can expect as a result and what is not guaranteed (whitening above all); and the dentist's name and the date of signature.
In Saudi Arabia regulators check that consent exists in Arabic; a form in Arabic and English together answers the question for non-Arabic-speaking patients.
When it is signed: before the appointment, not in the chair
A patient who signs in the chair with the anaesthetic in hand has read nothing, and consent signed under pressure is weak consent. Better for the form to arrive with the appointment confirmation on WhatsApp, so the patient reads it at home, signs from the phone, and arrives having already agreed.
Anyone who has not signed before the appointment signs at reception on a tablet before going in, not in the surgery. And the dentist gets an alert if a session starts with the form unsigned.
Where it is stored: on the procedure, not in a folder
The signed form belongs to the case it covers, not to a general attachments folder or a drawer. When it is stored on the procedure itself in the patient file with the x-rays the decision was based on, "where is the extraction consent?" two years later is one click.
The electronic signature is stored with its time, device and the version of the form as the patient saw it, which is what is asked for in any dispute.
Financial consent is part of consent
For orthodontics, implants and large restorations, the financial agreement (the amount, the instalments, what happens on drop-out) is signed with the clinical consent or as part of it. A clinic with both signed does not get into an argument at instalment ten or at a complication.
The clinic writes and updates its forms
The wording of consent is a clinical and legal decision for the clinic, not text imposed by the vendor. The software provides the templates; the clinic writes its own text, approves it with its adviser, and updates it when something changes, keeping the old version because a patient who signed a year ago signed that version.
What to ask of the software
A form per procedure in two languages, sent with the confirmation and signed from the phone or on the reception tablet, stored on the procedure in the patient file with time, device and text version, an alert when a procedure starts without consent, and form versions kept by date.

Questions
Is a general consent form at registration enough?
No. Informed consent is specific to the procedure, its risks and its alternatives. A general form proves the patient registered, not that they understood what would happen in their mouth.
Is an electronic signature from the phone acceptable?
The signature is stored with its time, device and the form version the patient saw, which is what is required in most cases. Check with your legal adviser for your country's requirements and the type of procedure.
Who signs for a child?
The guardian registered on the child's file, with their name and relationship stored with the signature. The form goes to their phone, not the child's.
What if the patient refuses to sign?
The refusal is recorded on the file and the dentist decides whether to proceed. For surgical procedures the answer is usually no.
Does whitening need a form?
Yes, because it is the procedure with the most complaints about results. The form sets out what to expect, what is not guaranteed, and temporary sensitivity.
How do we find a form signed two years ago?
From the patient file, on the procedure it covers, with the text version they signed at the time rather than the current one.
