Dental consent form generator

Eleven dental consent forms, each with the treatment explained, its benefits, common and rare risks, the alternatives and the patient’s declarations. Pick the teeth on a chart, adapt the wording to your practice, and let the patient sign on screen or on paper.

How to use it

  1. Pick the treatment

Simple or surgical extraction, implant, root canal, fixed braces, aligners, whitening, crowns and bridges, local anaesthesia, a child’s treatment, or photographs and records. Or start from a blank form.

  2. Name the teeth

Tap them on the chart or type them in FDI, Universal or Palmer. The form prints the number, and for up to three teeth the tooth’s name as well.

  3. Make the wording yours

Every section can be edited in Arabic, English or both. Save the edited form as your clinic’s version for next time.

  4. Sign and print

The patient, or a parent for a child, signs with a finger or the mouse, or on paper after printing. Then save the PDF.

Why a dental consent names the tooth

A dental consent often has to settle not whether a risk was mentioned but which tooth the patient agreed to, when they remember one and the notes say another. A form that says “extraction” and nothing more cannot answer that. These forms print the tooth number in the system your clinic uses, the tooth’s name in words the patient can read, and, if you want it, a small chart with the tooth filled in.

The same goes for the risks that are particular to the mouth: the nerve beside a lower wisdom tooth, the sinus above an upper molar, the crown a tooth may need after root canal treatment. They are written into each form as plain points, ready for you to edit rather than to write from nothing.

A template, not legal advice

Consent requirements differ between Saudi Arabia, the Gulf, Egypt, Iraq and Jordan, and between a general practice and a specialist one. Every form says on its face that it should be reviewed with your legal adviser and adapted to your practice and local regulations before use, and no medicine is named or dosed anywhere on it.

Save your design

Without saving, your clinic details, design and templates are cleared when the page reloads. Leave your email and they are kept in this browser from now on, and we will show you how they work in a Dentolize account, on every device and attached to patient files.

The design and clinic details are kept in your own browser only. We receive your email and clinic name only: not the logo, not the signature and nothing about any patient.

Questions

Is the dental consent form generator free?

Yes, with no sign-up and no watermark on what you print.

Which dental treatments are covered?

Simple extraction, surgical extraction and wisdom teeth, implant placement, root canal treatment, fixed braces, clear aligners, tooth whitening, crowns and bridges, local anaesthesia, children’s treatment with a parent’s consent, and photographs and records. A blank form covers anything else.

Are the risks already written?

Yes, as general starting points: common risks and rare but serious ones for each treatment. Edit them to match how you work, or clear them and the headings print with lines to write on.

How does the children’s form work?

It is worded for a parent or legal guardian, prints a guardian section with their name, relationship and signature, and leaves an optional line for an older child to sign too.

Is the patient’s signature saved?

No. It goes into the document you print or save as a PDF and is cleared when you close the page. Nothing about the patient, including the teeth you pick, is sent to us.

Is this form legally valid?

That depends on your country and the treatment. Use it as a structured draft and review it with your legal adviser before you rely on it.

Consent signed at the chair, and kept

In Dentolize the patient signs on a tablet at the chair or on their own phone, and the consent is kept on their file with the version of the wording they signed. Thirty minutes on your own clinic.
