Printable dental chart
A dental charting sheet on your own letterhead: adult, child or mixed dentition, tooth outlines with the five-surface squares, perio boxes if you want them and a legend of the usual symbols. Print a blank pad, or one for a patient with the missing teeth already crossed out.
How to use it
Set up your letterhead once
Clinic name, logo, address and phone in the Letterhead tab, shared with every other tool.
Choose the chart
Adult, child or mixed teeth; FDI, Universal or Palmer numbering; tooth outlines, the five-surface square or M O D B L boxes; perio boxes on or off.
Choose the page
A full exam record with patient details, medical alerts, a findings table and lines for notes, or a large chart on its own. Two charts, for the existing condition and the plan, if you want them.
Print or save the PDF
Leave the patient fields empty for a pad of blank charts, or fill them in and tap the missing teeth for one patient. Black lines on white, so it photocopies.
How to fill in a dental chart
The chart is drawn as you face the patient: the patient’s right on your left, upper teeth on top with their roots pointing up, lower teeth underneath. Each tooth has its number, an outline of the crown and roots, and a five-surface square: occlusal, or incisal on a front tooth, in the middle; buccal on the side towards the cheek or lip; lingual or palatal on the side towards the tongue or palate; mesial towards the midline and distal away from it.
Mark each finding on the surface where it is: shade a carious surface, fill in an existing restoration, ring a crown, draw a line down the root of a root-filled tooth and cross out a missing one. Most clinics use red for work needed and blue or black for work already done; the legend’s shapes keep them apart on a black-and-white copy too.
Existing condition and treatment plan
Tick Two charts and the sheet carries one chart of the mouth as you found it and a second for what you propose. Keeping them apart means the record of the examination is never overwritten by the plan, and that record is what a colleague, an insurer or a complaint will ask to see.
Perio boxes add mobility and pocket depths on the cheek and tongue side of every permanent tooth: enough for a basic periodontal screen at a check-up, not a full six-point chart.
Save your design
Without saving, your clinic details, design and templates are cleared when the page reloads. Leave your email and they are kept in this browser from now on, and we will show you how they work in a Dentolize account, on every device and attached to patient files.

Questions
Is the dental chart free to print?
Yes, with no sign-up and no watermark. Print it as often as you like, or save the PDF and print a pad.
Can I print a child’s dental chart?
Yes. Choose Child for the 20 primary teeth, or Mixed to show the primary teeth inside the permanent ones, each under the tooth that will replace it.
Does it use FDI or Universal numbering?
Either, and Palmer too. FDI is the default; change it in the Chart tab. To check a single number, use the tooth numbering converter.
Will it print well in black and white?
Yes. The chart is black line work and the symbols differ by shape, not colour, so it photocopies cleanly. Only the letterhead uses your colour.
Can I mark missing teeth before printing?
Yes. Tap them on the small chart in the Chart tab and they print crossed out. Like the patient’s name, they are not saved anywhere.
Is any patient information stored?
No. The name, the medical alerts and the missing teeth stay in the page while you work and are gone when you close it. Only your letterhead and chart settings are kept, in this browser, if you choose to save.
The chart that becomes the plan
In Dentolize you chart on screen, tooth by tooth and surface by surface, and every planned treatment turns into the plan, the quote and the invoice. Thirty minutes on your own clinic.
